INSTRUCTIONS

Note: This questionnaire is very long. Set aside about 30 MINUTES
to an HOUR to complete it. Really.

PLEASE read these instructions BEFORE proceeding with the
questionnaire.

1. Delete everything above "START OF QUESTIONAIRE" and below
"END OF QUESTIONAIRE". Then either check with an "x" or
"X" the appropriate "[ ]" like this:

Leave inapplicable or overly personal questions blank. If you
are so inclined, you may also delete responses that are not X'd
out, BUT be sure to X your responses.

Note: Please stick to this format, since I can't guarantee that
I will have time to hand process forms that don't use it.

2. If you have suggestions for alternate responses, or you just
want to make some comments, please consider using some sort of
tag. Many past responders have used "***" at the beginning of
the comment, and I have found that to be very helpful.

I'll run this for about 5 weeks, starting the week of 20 October,
2004, and ending at 23:59 CST on Wednesday 24 November, 2004.
However, notices of the survey will be posted weekly in
rec.arts.anime.misc and rec.arts.anime.fandom for 3 weeks, and then
the survey will be opened to web forums at the end of the third week
for two more weeks.

After the closing date, I'll post the results in most of the
rec.arts.anime.* groups as well as on my web site at
